In order to enroll in an independent study course, please complete the
information required below.  Be sure to indicate the exact course designation
for which you want to receive credit and the faculty member under whose
supervision you will work.  After securing the approval of this faculty
member, bring this form back to the CS Division Office, 571 Evans Hall.
Please note that this form must be submitted each semester in which you wish
to enroll in an independent study course.

CS 99, 199 and 300 must be taken on a P/NP basis.  CS 299 courses are
normally taken on an S/U basis, but they may be taken for a letter grade, at
the option of the faculty member in charge of the course.  The S/U option is
recommended by the Departmental Graduate Study Committee unless extenuating
circumstances exist -- for example, in the final semester of a student's
research project when a more objective basis exists for assigning a letter
grade.
